Time Off Benefits

TIME-OFF & HOLIDAYS WHO PAYS FOR IT? WHEN AM I ELIGIBLE? WHAT DO I RECEIVE?
Company Holidays ARK Solutions, Inc. On Hire. Salaried employees working at client site shall follow client holiday schedule & all other employees are typically eligible for seven public holidays.
Personal Holidays ARK Solutions, Inc. On Hire. Salaried Employees: PTO increases with the tenure of employment. A maximum of one week paid vacation may be carried over from one calendar year to the next. Employee will lose any unused PTO over a week at the end of each year.
PTO Accruals as of January 1st, 2015:
  • 1st year12 days
  • 2nd Year15 days
  • 3rd Year15 days
  • 4th Year21 days
Bereavement Leave ARK Solutions, Inc. On Hire. Eligible salaried employees may take two days off in the event of a death of an immediate family member.
Jury Duty ARK Solutions, Inc. On completion of 12 months of employment. Employees receive a maximum of 1 week of jury duty to be served in a calendar year and are expected to report for work whenever the court schedule permits.
FMLA ARK Solutions, Inc. On completion of 12 months of employment. Employee receives a total of 12 weeks of unpaid leave in a calendar year for the following reasons:
  • Birth and care of a new-born child or adopted child.
  • Care for an immediate family member with serious health conditions.
  • Employee suffering from serious health conditions.
 

Health Insurance

HEALTH INSURANCE WHO PAYS FOR IT? WHEN AM I ELIGIBLE? WHAT DO I RECEIVE?
Medical Insurance (Including Prescription Drugs) Eligible salaried employees: ARK Solutions, Inc. contributes 80% towards employee premium (effective 12/01/2014) and 25% towards dependent premium. Non-salaried employees: Can take advantage of the group rate. Both plans include a three-tiered prescription plan. On Hire United Health Care (UHC):
  • Choice Plan (HMO Plan)
  • Choice Plus Plan (PPO Plan)
Dental Insurance Eligible salaried employees: ARK Solutions, Inc. contributes 80% towards employee premium (effective 12/01/2014) and 25% towards dependent premium. Non-salaried employees: Can take advantage of the group rate. Both plans include a three-tiered prescription plan. On Hire UHC dental PPO plan is available.
Vision Insurance Employee. On Hire UHC Vision plan. (Employees can take advantage of the group rate.)
Benefits Premium Pre-Tax Option N/A On Hire Opting for the pre-tax option allows the employee to reduce taxable income by paying the employee cost share of premium before tax.

Term Life Insurance ARK Solutions, Inc. On completion of 30 days of employment. Salaried employees are eligible for a flat coverage of $50,000/-.
 

Disability Insurance

DISABILITY INSURANCE WHO PAYS FOR IT? WHEN AM I ELIGIBLE? WHAT DO I RECEIVE?
Short-Term Disability ARK Solutions, Inc. On completion of 12 months of employment. Salaried employees will be eligible for 60% of weekly earnings to a maximum of $1000/- per week. Employees are eligible for this benefit 14 days after disability. Duration of benefit:
  • 11 weeks
  • 6 weeks (For Maternity)
Long-Term Disability ARK Solutions, Inc. On completion of 12 months of employment. Salaried employees will be eligible for 60% of monthly earning to a maximum of $5000/- per month. Employees are eligible for this benefit 90 days after disability.
